If you’ve had a positive home pregnancy test, please complete our Client Intake Form and our administrative staff will contact you. Complete this form for each new pregnancy, whether you’ve been a previous client or not.

URGENT Concerns: Registered clients, please page your midwife at the pager number that was provided in your first appointment.

If you require medical attention before your first midwife appointment, please contact your family doctor or go to the nearest emergency department.
